YCCO is searching for ONE (1) full-time CARE MANAGER who is one of the following:
SOCIAL WORKER (CSWA / LCSW) OR NURSE (LPN / RN)

Reports To: Care Management Manager Physical Strength: Light (L)
Location: Hybrid. Most of the positions at YCCO are hybrid, meaning they have the flexibility to work both remotely and/or in-person at YCCO's office in McMinnville, OR.
Care Manager Social Worker Position:
Summary
Under the direction of the Care Management Manager and the Nursing Director - RN, the Care Manager Social Worker (CM - SW) performs outlined Care Management duties for priority population members, members with behavioral health issues, and chronically ill Medicaid members. The CM – SW offers social work intervention and coordination of needed services to members and/or families experiencing psychosocial adjustment difficulties associated with illness, injury, disability and related hospitalization and/or health care interventions. The CM - SW works in collaboration and continuous partnership with members, their families / caregivers, clinical support staff, clinic providers, and community resources in a team approach to improve member outcomes and increase the member’s ability to self-manage.
Education & Experience: Care Manager Social Worker
Required:
- Master’s in Social Work with a Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW).
- Minimum 1-3 years’ work experience in healthcare setting including experience with care management, disease management, and/or quality improvement.
- Minimum 2 years' clinical experience.
OR:
- Any combination of education and experience that would qualify candidate for the position.
Preferred:
- Experience with mental health / addiction disease services (strongly preferred).
Certificates, Licenses, and/or Registrations
Required:
- Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW).
- Certified Care Manager (CCM).
OR:
- Achievement of certification (CCM) within 12 – 24 months of hire.
Care Manager - Nurse (LPN / RN) Position
Summary
Under the direction of the Care Management Manager and the Nursing Director - RN, the Care Manager - Nurse (CM - Nurse) performs outlined Care Management duties for priority population members, members with behavioral health issues, and chronically ill Medicaid members with diagnoses that include chronic kidney disease, diabetes, chronic obstructive pulmonary disease (COPD), and cardiovascular disease. The CM - Nurse works in collaboration and continuous partnership with members, their families / caregivers, clinical support staff, clinic providers, and community resources in a team approach to improve member outcomes and increase members ability to self-manage.
Education & Experience: Care Manager - Nurse (LPN / RN)
Required:
- Graduate from accredited school of nursing.
- Minimum 1-3 years’ work experience in healthcare setting including experience with care management, disease management, and/or quality improvement.
- Minimum 1-2 years' clinical nursing experience in an area such as medical / surgical, skilled nursing, or behavior health.
OR:
- Any combination of education and experience that would qualify candidate for the position.
Preferred:
- Bachelors in science in Nursing (BSN).
- Experience with mental health / addiction disease services (strongly preferred).
Certificates, Licenses, and/or Registrations
Required:
- Valid, unrestricted Oregon LPN / RN license.
- Certified Care Manager (CCM).
OR:
- Achievement of certification (CCM) within 12 – 24 months of hire.
